ACCIDENT / INCIDENT INVESTIGATION AND REPORTING
ONSITE CONSULTATION OCCUPATIONAL SAFETY & HEALTH
Onsite consultation means the provision of consultative assistance on an employer's occupational safety and health program and on specific workplace hazards through a visit to an employer's worksite. It includes a written report to the employer on the findings and recommendations resulting from the visit.
ACCIDENT / INCIDENT INVESTIGATION AND REPORTING
To explain why and how an accident happened, investigators must collect information on the events that took place before and during the event.
In any business or organisation things don't always go to plan. You need to prepare to deal with unexpected events in order to reduce their consequences. Workers and managers will be more competent in dealing with the effects of an accident or emergency if you have effective plans in place that are regularly tested.
You should monitor and review any measures you have put in place to help control risk and prevent accidents and incidents from happening. Findings from your investigations can form the basis of action to prevent the accident or incident from happening again and to improve your overall risk management. This will also point to areas of your risk assessments that need to be reviewed.
An effective investigation requires a methodical, structured approach to information gathering, collation and analysis.

In warehouses with confined space requirements, narrow aisle configurations make it difficult to operate traditional forklifts safely and effectively. In these situations, OSHA requires the use of reach trucks, i.e. reach lifts or reach truck forklifts.
What Is a Reach Truck?
A reach truck is a type of forklift designed specifically for narrow aisles. The truck has wheels that are located directly below an operator to create a tight turn radius. In doing so, a reach truck helps an operator navigate tight spaces and reach high racks with minimal risk.
Merely providing warehouse workers and others who complete tasks in tight spaces is insufficient. Because, if workers do not know how to operate a reach truck, they could crash into aisles or cause other accidents. Perhaps even worse, these accidents could lead to on-the-job injuries and fatalities.
How to Operate a Reach Truck
To determine how to properly operate a reach truck, consult with the owner’s manual. This provides an operator with instructions relating to truck acceleration and deceleration, programmability, and other functions. It also lets an operator know how to take care of a reach truck and minimize the risk of maintenance issues.
Along with reading the owner’s manual, reach truck operator certification is crucial. A reach truck is a distinct type of forklift, and it must be operated with care, precision, and attention to detail. If forklift operators complete reach truck operator certification training, they can learn how a traditional forklift differs from a reach truck. Plus, these operators can learn how to safely use both types of forklifts.

Scissor lifts are work platforms used to safely move workers vertically and to different locations in a variety of industries including manufacturing, retail, and construction. Due to their versatility, they provide convenience to employees by eliminating the additional need of scaffolding or ladders to perform common tasks.
Scissor lift safety is critical and depends on considering equipment capabilities, limitations and safe work practices.
This course is designed to familiarize you with the different types of hazards related to scissor lifts, as well as help you understand what preventive measures you can take to stay safe while operating or working near them.

Vehicles help make our lives easier, from getting us to where we need to go, to helping us transport something. While all vehicles present potential hazards, some are more dangerous than others. A good example is the concrete mixer truck.
Understanding safe practices around concrete mixer trucks are vital for protecting lives. Anyone involved with operating a concrete mixer truck should be properly trained and follow all pertaining safety procedures and standards.
OSHA has published safety tips regarding vehicles like concrete mixer trucks. These are just some safety practices that relate particularly to vehicle safety:
-
Make sure back-up alarms on all vehicles are functioning.
-
Use care with the load out chute on concrete mixers to avoid injuries to hands and fingers.
-
Beware of hot surfaces on equipment and truck components.
-
Guard eyes against splashes of aggregate materials during loading and unloading.
-
Use hearing protection if needed to guard against excessive noise exposure during cement loading/unloading and while using pneumatic chippers inside truck mixer drums.
Concrete Mixer Truck Safety
Be sure that trucks and other vehicles are in good working order, including audible back-up warning signals, before operating them.
Check for overhead clearance while driving and before unloading the truck.
Because the concrete truck’s center of gravity shifts frequently while driving, take turns slowly and brake softly.
Be mindful of blind spots while backing up.

This brief guide describes what you, as an employer, need to do to protect your employees from falls from height. It will also be useful to employees and their representatives. Following this guidance is normally enough to comply with the Work at Height Regulations. You are free to take other action, except where the guidance says you must do something specific. Falls from height are one of the biggest causes of workplace fatalities and major injuries. Common causes are falls from ladders and through fragile roofs. The purpose is to prevent death and injury from a fall from height. Work at height means work in any place where, if there were no precautions in place, a person could fall a distance liable to cause personal injury.
For example you are working at height if you:
■ are working on a ladder or a flat roof;
■ could fall through a fragile surface;
■ could fall into an opening in a floor or a hole in the ground.
Take a sensible approach when considering precautions for work at height. There may be some low-risk situations where common sense tells you no particular precautions are necessary and the law recognizes this. There is a common misconception that ladders and stepladders are banned, but this is not the case. There are many situations where a ladder is the most suitable equipment for working at height. Before working at height you must work through these simple steps:
■ avoid work at height where it is reasonably practicable to do so;
■ where work at height cannot be avoided, prevent falls using either an existing place of work that is already safe or the right type of equipment;
■ minimize the distance and consequences of a fall, by using the right type of equipment where the risk cannot be eliminated.

The course aims to increase awareness and impart basic knowledge on occupational ergonomics and related ergonomic related disorders (e.g. carpal tunnel syndrome, back injury) with focus on manual handling at the workplace. The training would assist the participants to evaluate their workplace and improve working conditions and ways of doing things so as to work comfortably and efficiently.
Workplace ergonomics is the science of designing the workplace, keeping in mind the capabilities and limitations of the worker. Poor worksite design leads to fatigued, frustrated and hurting workers. This rarely leads to the most productive worker. More likely, it leads to a painful and costly injury, lower productivity and poor product quality.
A workplace ergonomics improvement process removes risk factors that lead to musculoskeletal injuries and allows for improved human performance and productivity.

SCHEDULE WASTE AND SPILL MANAGEMENT TRAINING
Many businesses use toxic and hazardous materials in their day-to-day activities. Many of these activities produce wastes that can injure or even threaten living things. These wastes are called hazardous or scheduled wastes. Scheduled waste must be handled in special ways to prevent threats to human health and the environment.
When disposed of in the municipal landfill or otherwise improperly managed, these materials have the potential of contaminating the ground water – our drinking water. Improper management of such wastes will lead to serious pollution of the environment and the ecosystem as well as immediate /long term impacts to human health.
Thus, the management of scheduled waste is a growing concern in Malaysia due to increase in the number of non-compliances and illegal dumping cases. Managing scheduled wastes requires a comprehensive understanding of regulatory framework, waste properties, waste management concepts, and accepted good management practices.

Accident / Incident Investigation is a technique, adopting which the Investigation or Investigating Team proceed procedural, sequentially and analytically to obtain details, circumstances, reasons and facts behind the occurrence, based on which the immediate causes and more importantly the root causes are ascertained. This technique has to be learnt through training and field exercises so that the trained/ skilled persons help the organization in reaching the facts behind the occurrence. The recommended measures help in preventing such or even more serious incidents in future.
Trainees also come to learn the legal, financial, moral, societal and technical reasons for conducting investigation. It is worth noting that for significant incidents, there is high power judicial/ technical investigation that forms the basis for changes in law of the land.
BENEFITS OF ACCIDENT INVESTIGATION TRAINING
-
The causes behind the occurrence help in formulating measures to prevent such or even more serious incidents.
-
Organization is equipped with a band of trained investigators who can shoulder the task of conducting inquiry investigations.
-
The organization can define and chalk out more efficient roles and responsibilities.
-
Managerial skill and work force behavior can be further improved based on the positive recommendation of the investigations.

This program is intended as an overview of the basic elements of hearing conservation to include the methods of selection, use, and care of hearing protection. Today, more than 20 million workers are exposed to hazardous noise on and off the job. An effective hearing conservation program is designed to prevent hearing loss. Be sure to do your part and wear the appropriate hearing protection to protect your ability to hear for a lifetime.
Awareness in Occupational Health is one of crucial agenda that requires special attention according to the Occupational Safety & Health Act 1994 (OSHA). Most organization are aware of this and include the importance of complying the acts in their company’s policies. However due to some reasons, many have failed to educate and train their employees and contractor into better performing in their OSH roles and responsibilities. As a result, many organizations have failed to control OSH standards and the place of work causing increases of industrial diseases.
HEARING CONSERVATION PROGRAM REQUIREMENTS
If monitoring data indicates that you need a hearing conservation program, there are additional steps that you need to take to protect your employees and comply with OSHA regulations. The following are brief descriptions of an employer’s responsibilities for a hearing conservation program, and are not meant to be a comprehensive listing of said responsibilities.
All employees exposed to decibel levels requiring the hearing conservation program must be notified of the results of the monitoring. They must also be allowed to observe the noise measurements that are conducted.

Countless companies in the manufacturing and construction industries rely on overhead cranes to lift and transport materials. When installed and used properly, these systems make operations easier and safer. But, overhead crane accidents cause severe injuries and fatalities every year. Preventing these disasters requires workers to recognize certain hazards that occur during operation and follow safety procedures to avoid them.
There are multiple hazards that can arise regarding cranes in general. Many accidents involve large lift systems like tower cranes and mobile cranes. But hazards do exist with all types of cranes including overhead cranes and in all facets of crane operation. (Overhead cranes are defined by OSHA as a crane with a movable bridge carrying a movable or fixed hoisting mechanism, and traveling on an overhead fixed runway structure.)
Analysis of overhead crane accidents reveals three common safety hazards that every company using overhead lift systems should be aware of to keep their workers safe. It’s important to be familiar with these hazards and learn to recognize them in the workplace in order to avoid them. The three most common hazards involving overhead cranes include electrical hazards, overloading, and materials falling/slipping from overhead hoists.
The following analysis of each hazard provides a description, potential risks, reasons why accidents occur, preventative measures to avoid them, and applicable OSHA requirements. One commonality that all three hazards share is the qualifications of crane operators. It is the responsibility of the crane owner and job supervisor to ensure that crane operators are competent and qualified to do the job. Click here to read more about training requirements and minimum competent personnel guidelines outlined by OSHA.
Safety Tips for Overhead Crane Operation
1) Before use, ensure the crane is suitable for the planned hoisting task. Confirm it has appropriate travel, lift, and capacity.
2) Visually and physically inspect the crane before use. Check for damage, wear, and proper operation of all functions.
3) Confirm the load weight. Check the capacity of all equipment including the hardware, rope, and slings. Do not exceed these capacities.
4) Select the right sling for each lift. Inspect slings and other rigging hardware before use for wear, stretch, or other damage. Do not use damaged or defective slings. Use softeners around sharp corners. Do not splice broken slings.
5) When communicating with a crane operator, use clear agreed-upon signals. Except for the stop signal, the crane operator should follow instructions from only one person – a designated signaler. Where a wired or remote controller is used, the operator should become familiar with all of its functions before lifting the load.
6) Warn all people in the load lift area before starting the lift. Ensure that the path of the load is clear of persons and obstructions. Do not lift loads over anyone. Center the crane hoist over the load before hoisting to prevent swinging of the load.
7) Slide the sling fully onto the hoisting hook and ensure the safety latch is closed. Do not load the hook tip or hammer a sling into place.
8) Secure unused sling legs. Do not drag slings or leave loose materials on a load being hoisted.
9) Keep hands and fingers from being trapped when slack is taken out of a sling. Step away before the lift is started.
10) Move the load and controls smoothly. Minimize load swing.
11) Walk ahead of the load during travel and warn people to keep clear. Use a tagline to prevent rotation or other uncontrolled motion. Raise the load only as high as necessary to clear objects. Do not ride on the hook or load.
12) Set loads down on blocking, never directly on a sling. Do not pull or push loads out from under the hoist.
13) Do not leave the load (or the crane) unattended while the load is suspended.
14) Where crane operation by other personnel must be restricted, employ lockout and tagging procedures.
15) Store slings off the floor in a clean, dry location on hooks or racks. Do not leave slings, accessories, or blocking lying on the floor.

Boilers are an important asset used for various purposes in different industrial plants. It’s one of the main application to produce high-pressure steam for process applications and power generation. Operation and maintenance of Boiler are critical to ensure a safe and efficient plant environment.
Boiler training ensure participants gain a comprehensive understanding of commercial, industrial and utility boiler systems. This program covers the principle of operation of steam boilers, types and the main components of steam boilers. It will also focus on the study of correct operation and routine maintenance and safety. Participants will also learn how to improve boiler efficiency, energy cost saving methods and maximise safety.
Reliable, efficient, environmentally friendly, and used in nearly every industry, boiler systems have the ability to generate energy, heat buildings large and small. But they can also be dangerous IF EMPLOYEES ARE NOT PROPERLY TRAINED IN THEIR OPERATIONS, MAINTENANCE, AND SAFETY.
Each year, boilers malfunction. Often, this results in explosions, the force of which is equal to that of a bomb, according to the NATIONAL BOARD OF BOILER AND PRESSURE VESSEL INSPECTIONS. Sometimes these malfunctions are the result of defect boilers themselves. Often they are the result of improper maintenance, a dearth of inspections, or safety oversights.
Many of these explosions could likely be avoided if employees had the proper training and a clear understanding of the potential dangers of malfunctioning or unmaintained boiler systems.
